  2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Support and Maintenance Agreement to include recalibration of all Lab External Sensors for one year in Lubbock and Amarillo.

  1. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Check Point System was bought in late 2008 so that the station could be in compliance with CO & JCAHO mandate to monitor temperature readings in their critical areas. The current equipment that was bought was designed by this company and can only be monitored by this company. It would be cost prohibited to purchase new equipment and change vendors at this point.

  1. Description of market research conducted and results or statement why it was not conducted:

Market research confirms there is No authorized SDVOSB's/ VOSB's to perform maintenance and repair on these products. It is in the best interest of the government to this procurement directly to the OEM EOI, Inc. dba Medical Resources, a WOSB.

Based on the market research conducted, no two or more VIP verified and capable SDVOSBs or VOSBs were identified and appeared capable of performing the requirement. In addition, no small business in any of the other targeted socio-economic groups appeared to qualify for a set-aside or sole source procurement.

1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
