Contractor and Subcontractor are each a "Party" and collectively are the "Parties." Terms used in this Modification will have the meanings given to them in the Agreement unless otherwise defined below. The efforts of Subcontractor under the Agreement as modified by this Modification will support the Prime Contract No. HHSM-500-2016-00067C ("Prime Contract ), dated September 28, 2016, issued by The Centers for Medicare & Medicaid Services ("CMS"). This Modification modifies the following items: 1. This Modification extends the Agreement for the Option 4 Period below: Description Period of Performance Option Period 1 3/28/2019-6/27/2019 Exercised Option Period 2 7/1/2019-7/31/2019 Exercised Option Period 3 8/1/2019-8/31/2019 Exercised Option Period 4 9/1/2019-3/31/2020 Exercised 2.
