1. De-scope task C.1
healthcare-associated infections
(HAIS) and transitioning the hai
work to the hiins.
2. Expand task C.3.6 for ade data
collection and support - - existing
recruited communities in cohorts
a, B, and C in the C.3 care
transitions project will be
encouraged to form and activate
medication safety workgroups if
they have not already done so.
Interventions may include: ade
screening and reduction,
medication reconciliation, and
others. An important area of focus
will be finding and leveraging
consumer and family engagement
interventions and strategies
promoting medication safety and
awareness of adverse drug events.
3. New subsection C.3.10 for
microbial stewardship - the
planned approach will target the
outpatient provider settings in our
states to advance care management
approaches in microbial
stewardship through the core
elements. Gpqin will provide
education on the core elements of
antibiotic stewardship in the
outpatient setting once available,
along with individualized technical
assistance on processes to
implement the core elements.
4. Merge existing work under task
B.4 health it and task D.1 quality
reporting - gpqin will use a
four-pronged strategy to provide
customized technical assistance to
eligible clinicians at the level in
which they need it, at the time they
need it, and that provides the
greatest value to CMS with the least
amount of intervention and cost
incurred.
