Background
The West Virginia Department of Health Facilities is soliciting bids to establish a contract for Physician Services to the residents of Jackie Withrow Hospital, located at 105 S Eisenhower Dr. Beckley, WV 25801.

This contract aims to provide essential medical services to long-term care residents, ensuring their health and well-being. The funding for this contract may include federal dollars, and vendors must comply with the Federal Funds Addendum.

Work Details
The contract requires the provision of Medical Director and Physician Services, which includes:
1. Monthly visits to each assigned resident for the first 90 days of admission and every 60 days thereafter.
2. On-site Physician Services at least one day per week (minimum 8 hours) and on-call services 24/7.
3. Attendance at quarterly Quality Assurance Meetings and mortality reviews as requested by the facility.
4. Responsibility for approximately 65 Long Term Care Residents, including potential TB residents.
5. Documentation in electronic medical records during service delivery.
6. Training on the facility’s Electronic Medical Record System.
7. Authority to admit residents who meet admission criteria.
8. Billing and collection from Medicare, Medicaid, or Third Party Insurances for services rendered.
9. Compliance with mandatory training and background checks.

Period of Performance
The contract will be effective upon award and will continue until all services are rendered or until terminated by either party with appropriate notice.

Services will be performed at Jackie Withrow Hospital located at 105 S Eisenhower Dr., Beckley, WV 25801.
