Concrete Pad, Fencing and bollard Replacement

Background
The West Virginia Office of Shared Administration, Purchasing Division is soliciting bids on behalf of the Department of Health Facilities (DHF), Welch Community Hospital (WCH) for a contract to construct one concrete pad and install fencing and bollards around the concrete pad. This project is partially funded by federal funds, and all bidders must acknowledge and adhere to the attached Federal Funds Addendum.

Work Details
The contractor will be responsible for the following tasks:

1. Removal of existing fences, concrete pad, and bollards, including all related debris which must be disposed of in accordance with federal, state, and local laws.

2. Installation of one concrete pad with a monolithic pour as per provided drawings (Exhibit C).

3. Installation of approximately 19 bollards made from 6" diameter schedule 40 steel pipe, minimum 8 feet in length, buried at least 48 inches into the ground with specific footing requirements.

4. Installation of approximately 12 feet of commercial-grade chain link fencing that is 96 inches high with barbed wire on top and a gate with specified dimensions.

5. Final cleanup activities prior to final inspection.

Place of Performance
The work will be performed at Welch Community Hospital in West Virginia.

Bidder Requirements
Bidders must comply with various requirements including holding a valid West Virginia contractor's license, providing a bid bond of at least 5% of the total bid amount, and adhering to regulatory standards such as the West Virginia State Building Code and other applicable codes.
