Description

Background
Dane County is seeking a provider to partner with onsite property management to deliver supportive behavioral health services to residents of Enso Apartments. This facility consists of 60 studio apartments located at 715 Rethke Ave, Madison, WI 53714. The goal of this contract is to enhance the well-being and stability of residents through targeted case management and crisis stabilization services.

Work Details
The contractor will provide supportive behavioral health services including:
- Targeted Case Management (TCM) for individuals with severe mental illness or substance use disorders.
- Crisis stabilization services.
- Coordination with onsite property management (Lutheran Social Services) to ensure residents receive comprehensive support.
- Development of individualized service plans focusing on maintaining permanent housing and connecting residents to medical and behavioral health resources.
- Engagement in community activities and facilitation of social support networks.
- Monthly reporting on service usage, resident satisfaction, and performance indicators such as tenancy retention rates and enrollment in Medical Assistance programs.

Period of Performance
The contract will be effective for one year from the execution date, with an option for mutual renewal for up to four additional one-year periods.
