Custom Seating Materials

Background
Central Wisconsin Center (CWC) is a state-operated residential facility for individuals with developmental disabilities, located in Madison, WI. CWC houses approximately 150 residents and operates under the Division of Care and Treatment Services of the Wisconsin Department of Health Services. The purpose of this solicitation is to procure custom seating materials for repairing existing wheelchair seats and creating customized seating for residents.

Work Details
The contractor will provide the following custom seating materials:

1. Boltaflex Colorguard vinyl 54" x 30yd roll, any color
2. Volara Type 2A (2#), 0.25" x 60" x 300' roll
3. Polyurethane foam HR 2850 1" x 24" x 96"
4. Polyurethane foam HR 2850 1.5" x 24" x 96"
5. Polyurethane foam 2530 1" x 24" x 82"
6. Polyurethane foam 2520 1.5" x 24" x 82"
7. 1/8" nylon backed neoprene 46" x 83"
8. Polyurethane Premium Antimicrobial Foam (Firm) - Sizes: 26”x48”, 24”x80”, and thicknesses of 1”, 2”, 3”, 4”, 5”, and 6”. Foam firmness: 60-75 lbs.
9. Polyurethane Premium Antimicrobial Foam (Medium) - Sizes: same as above with firmness of 40-59 lbs.

Period of Performance
The contract will be effective on the date indicated on the purchase order or contract and shall run for one year from that date, with an option to renew for two additional one-year terms by mutual agreement.

Place of Performance
Products will be delivered to Central Wisconsin Center, located at 317 Knutson Drive, Madison, WI.

Bidder Requirements
Bidders must have been in the business of providing similar materials for at least three years. Prices quoted must include all charges for transportation, packaging, insurance, etc., necessary for delivery. The contract cost shall not exceed $50,000.
