Hospital Transformation Technical Assistance

Background
The Agency of Human Services-Office of Health Care Reform is seeking to establish contracts with one or more companies to provide technical assistance support to hospitals for health care transformation. The State aims to enhance affordability, access, quality, and health outcomes through data-informed implementation and reporting. The selected vendor will assist hospitals in identifying opportunities and implementing initiatives for measurable progress in these areas.

Work Details
The vendor will meet with hospital transformation teams, review transformation plans, assess analytics and implementation support needs, and develop implementation plans with timelines tied to measurable outcome metrics. They will suggest high-impact transformation goals for hospital optimization and regional optimization while considering the full continuum of care.

The vendor will create evaluation matrices for successful implementations and sustainability plans focusing on quality, access, and cost metrics. They must have a deep understanding of healthcare systems, value-based payments, and population health management approaches.

Specific tasks include:
1. Support hospital decision-making through analysis of opportunities for improved efficiencies.
2. Produce actionable financial and operational recommendations.
3. Continuously improve approaches based on data feedback.
4. Align work with Vermont legislation and federal initiatives.
5. Develop implementation plans with clear timelines and milestones for all 14 hospitals in Vermont.
6. Provide standardized tools, templates, dashboards, and reports for tracking progress.

Period of Performance
The contract will be for one year with an option to renew for up to four additional twelve-month periods, starting by April 13, 2026.

Place of Performance
Services will be performed at various hospitals across the State of Vermont.

Bidder Requirements
Bidders must comply with specific requirements including: registration with the Vermont Secretary of State’s office; obtaining a Contractor’s Business Account Number from the Vermont Department of Taxes; adherence to worker classification compliance requirements if bid amounts exceed $250,000; completion of a Certificate of Compliance; and submission of a list of subcontractors prior to contract execution.
