Physical Therapy Services

Background
The Department of Behavioral Health and Developmental Services (DBHDS) is seeking sealed proposals for the purchase of Physical Therapy Services by the Southeastern Virginia Training Center (SEVTC), a State Intermediate Care Facility (ICF) in Chesapeake, Virginia. SEVTC provides education, habilitation, and health services to individuals with intellectual disabilities and their families in Health Planning Region V (HPR V). The center is committed to offering the highest possible quality of life to all individuals who reside in the SEVTC community.

Work Details
The contractor shall provide physical therapy services to individuals with intellectual or developmental disabilities at SEVTC. The physical therapist must be a graduate of an accredited physical therapy program and licensed to practice physical therapy in the Commonwealth of Virginia. The contractor shall provide general supervision of up to two full-time Licensed Physical Therapist Assistants (LPTA) and complete physical therapy evaluations, assessments, referrals, and related documentation for each individual admitted to SEVTC.
Additionally, the contractor must ensure quality care, provide transportation, orientation and training, work area, and comply with specific working hours, cancellation policies, and assignment extensions.

Period of Performance
The initial period of the contract is one year from March 1, 2024, through February 28, 2025, with four one-year renewal options.

Place of Performance
The services will be performed at Southeastern Virginia Training Center located at 2100 Steppingstone Square, Chesapeake, Virginia 23320.
