Perinatal Health Hubs

Background
The Virginia Department of Health (VDH) is issuing this Request for Applications (RFA) to solicit applications from eligible organizations to establish and/or expand perinatal health hubs in the Commonwealth of Virginia.

The goal of the Perinatal Health Hubs Pilot Program is to improve maternal and infant health outcomes by increasing access to comprehensive, culturally responsive perinatal support services, thereby reducing maternal and infant mortality and morbidity rates.

Work Details
Organizations will use awarded funds to implement one of the following categories:
1) Design and implement a new perinatal health hub;
2) Expand and improve an existing perinatal health hub; or
3) Establish or improve a collaborative perinatal health hub involving multiple organizations.

Each hub must provide a range of services including clinical services, community health worker support, care coordination, parenting education, lactation support, and assistance with applications for health insurance. Hubs must be community-based with a physical presence in the region they serve and tailored to meet local needs.

Period of Performance
The initial contract period will be from the date of award through June 30, 2026, with an option for a no-cost extension from July 1, 2026, to June 30, 2027.

Place of Performance
The contract will be performed within the Commonwealth of Virginia at designated community locations where the perinatal health hubs are established.

Bidder Requirements
Eligible organizations must be private entities (non-profit or for-profit) providing direct services to pregnant and postpartum populations based in Virginia.

Applicants must complete all required documentation by the submission deadline and demonstrate experience in serving underserved populations.
