Background
The Virginia State Police is seeking bids for a one-year term contract to print the Virginia State Uniform Summons. The purpose of this Quick Quote is to solicit unsealed bids for printing the summons at the VSP Warehouse located at 7700 Midlothian Turnpike, North Chesterfield, Virginia, 23235.

Work Details
The contractor is required to print the Virginia Uniform Summons in accordance with specific stock, size, and printing requirements. The form is a snap-apart, 4-part form with specific paper specifications for each part. The tear-down size should be 11” x 8 ½” with a padded stub along the right side and perforation for separation ease.

The contractor must print the form in black ink according to the most recent version provided by Virginia State Police. Additionally, the contractor must provide an electronic proof within 7 business days of request and deliver the forms shrink-wrapped in sets of 25, with specific labeling and packaging requirements. Dockside delivery is required no later than 35 calendar days after receipt of approved proof.

Period of Performance
The contract is for a one-year term with an estimated annual usage of 240 cartons and an estimated reorder each quarter. Each order will require an order minimum of 60 cartons.
