Description

State of Utah Division of Purchasing is the Issuing and Conducting Procurement Unit The purpose of this RFP is to enter into a contract to provide: Hazardous Material Courier Services primarily in the Eastern part of the State of Utah. This RFP may result in multiple awards. The methodology that will be used to determine the number of awards is: The methodology or formula that will be used to determine the number of contract awards is: Offerors who pass the mandatory minimums and achieve 70% of the technical threshold and 70% of the overall points (Technical + Cost) will be awarded.. Length of Contract: The contract resulting from this RFP will be for FIVE (5) years. Vendor must guarantee its pricing for ONE (1) Year. Administrative Fee: 0.00%.
Background
The State of Utah Division of Purchasing is issuing this Request for Proposals (RFP) to establish a contract for Hazardous Material Courier Services. The primary goal is to facilitate the timely transportation of laboratory specimens from various locations in eastern Utah to the Department of Health and Human Services for testing, as well as to other public laboratories and end users as needed. This contract aims to address the lack of courier services in the eastern part of the state.

Work Details
The contractor will provide courier services specifically for hazardous materials, including laboratory specimens. Key requirements include:
- Pickup and delivery of specimens on a weekly, semi-weekly, and on-call basis.
- Delivery of all laboratory specimens within 24 hours under appropriate conditions to preserve specimen quality.
- Delivery of tuberculosis specimens within 16 hours.
- Compliance with shipping regulations established by IATA and USDOT, as well as state and federal guidelines regarding hazardous materials transport.
- Coordination with the Department's client relations manager for any changes in locations or schedules with a two-week notice.

Period of Performance
The contract will be effective for a period of five (5) years, with pricing guaranteed for one (1) year.

Place of Performance
Services will primarily be performed in the eastern part of the State of Utah, with specific locations coordinated between the contractor and the Department of Health and Human Services.

Bidder Requirements
Bidders must meet several mandatory minimum requirements:
- Must have been in business for at least three years.
- Must possess all applicable licenses and certifications including U.S. D.O.T. 49 CFR 173.6, HIPAA certification, and bloodborne pathogen training.
- Must be able to obtain applicable federal clearances when required.
