(RFP) HCAAA Health Promotion Vision Services - P35

Background
The City of Houston is seeking qualified optometrists and vision suppliers to provide vision services and products for the Houston Health—Harris County Area Agency on Aging (HCAAA).

The HCAAA, established in 1977, provides federally funded social services for the elderly under the Older Americans Act. This contract aims to assist older adults in overcoming vision disorders that may lead to restricted reading abilities and isolation. The agreement will be for an initial one-year term with four one-year renewal options, totaling a maximum of five years.

Work Details
The contractor will provide comprehensive evaluations related to vision, education, counseling, and eyewear for eligible older adults aged 60 and above in Harris County. Services include:
- Routine Eye Exams: Pre-screening, refraction for glasses prescription, dilation for inner eye health.
- Provision of eyewear: Single-vision lenses, flat top bifocal lenses, trifocal lenses.
- Fitting of frames made from plastic (Optyl, Zyl) or metal (Monel, titanium).
- Follow-up counseling and educational information dissemination regarding medical eye care options.
- Completion of all vision-related services by the end of each fiscal year (September 30th).
- Submission of treatment plans and assessments to HCAAA after screenings.

Period of Performance
The contract will commence upon approval in March 2025 and will last for an initial term of one year with four optional one-year renewals, potentially extending the total duration to five years.

Place of Performance
Services will be provided within Harris County, Texas.
