- Providing culturally and linguistically appropriate outreach materials in multiple languages.
- Offering application assistance for healthcare services like the Medical Access Program (MAP) through one-on-one support.
- Making guided referrals to social and health resources including food, housing, and behavioral health services.
- Participating in regular coordination meetings with Central Health staff and submitting monthly activity reports along with invoices.
Period of Performance
The initial contract duration is one year with the option for four one-year renewal periods based on performance.

The contract will be performed primarily in Travis County and neighboring counties (Williamson, Hays, Bastrop), Texas.
