Health and Welfare Employee Benefits Insurance Plans (Medical/RX, & Dental)

Background
The City of Corinth, Texas, is seeking proposals for a licensed health insurance provider to manage its employee benefits plans, including medical, prescription drug (RX), and dental coverage. The goal is to furnish all necessary equipment, materials, supplies, labor, permits, insurance, and licenses to provide these services in accordance with the specifications outlined in the RFP. The City may award multiple contracts based on the proposals received.

Work Details
The selected contractor will provide comprehensive health insurance services for all eligible employees of the City of Corinth. This includes:

1. Medical Coverage: Fully insured medical plans including options for high deductible health plans (HDHP) and PPO plans.

2. Prescription Drug Coverage: Comprehensive RX plan details must be included in the proposal.

3. Dental Coverage: Full dental services including preventive, basic, and major services with specific coverage percentages outlined in the RFP.

4. Claims Handling: Quality customer service and prompt claims processing are required.

5. Continuity of Coverage: Immediate coverage under new plans starting October 1, 2026.

6. Monthly Reporting: Regular claims experience reports to be provided to the City’s Benefits Consultant.

Period of Performance
The contract will commence on October 1, 2026, with a duration of five years. Proposals may include options for rate guarantees or renewals.

Place of Performance
Services will be performed within the City of Corinth, Texas.

Bidder Requirements
Respondents must comply with all federal, state, and local laws regarding insurance and employee benefits. They must provide proof of insurance as specified in Appendix A and submit various forms including a Conflict of Interest Questionnaire and Form 1295 Certificate of Interested Parties. Additionally, vendors must have an A.M. Best rating of no less than A-VI.
