26025 Occupational Health, Injury, Drug and Alcohol Testing Services

The City of Grand Prairie is seeking proposals for Occupational Health, Injury, Drug, and Alcohol Testing Services to support its Human Resources Department. The services required include, but are not limited to:

- Occupational health assessments
- Injury-related medical services
- Drug and alcohol testing for employees

For a comprehensive overview of the requested scope of work and detailed service requirements, please refer to Attachment A located at the end of this solicitation packet.

The request for proposals is issued by the City of Grand Prairie.

Attachment A & B can be found at the bottom of the solicitation packet. All proposers must fully complete the Questionnaire included in the packet. In addition, the pricing sheet must be submitted in its original Excel format do not convert it to any other format.
