Nk Sr Ctr Multypurpose Rm Flooring Project Rfp

Background
The Town of North Kingstown, Rhode Island, through its Department of Public Works, is seeking qualified contractors to submit proposals for the Multipurpose Room Flooring Project at the North Kingstown Senior Center located at 44 Beach Street.
The project aims to improve the flooring in the multipurpose room to enhance functionality and aesthetics for community use.

Work Details
The contractor will be responsible for the following tasks:
1. Review existing conditions and perform field measurements.
2. Conduct moisture testing on the existing slab according to ASTM F 21470.
3. Remove and dispose of existing wood flooring while salvaging all bases and trims for later reinstallation.
4. Clean the floor to a surface profile of 3 after establishing proper RH & PH levels in the slab.
5. Apply a Moisture Mitigation system and a skim coat over the existing concrete floor.
6. Furnish and install a specified 5mm vinyl plank flooring, ensuring adhesive compatibility with the Moisture Mitigation System.
7. Provide a Lump Sum bid along with an alternate price for a floating flooring system and unit pricing for any base and trim replacements due to damage during removal.

Period of Performance
The contract period is expected to commence upon award and must achieve substantial completion by a date specified in the Bid Form.
Liquidated damages are set at $100 per day for delays beyond this date.

Place of Performance
All work will be performed at the North Kingstown Senior Center located at 44 Beach Street, North Kingstown, RI.

Bidder Requirements
Bidders must be licensed in Rhode Island for all trades required by this RFP and cannot subcontract any services without prior authorization from the Town.
A certificate of insurance showing $1 million General Liability and $1 million Any Auto coverage is required, with the Town named as an additional insured.
Bidders must also comply with RI Prevailing Wage requirements.
