The Public School Employees' Retirement Board's ( Board ) on behalf of the Commonwealth of Pennsylvania ( Commonwealth ) is seeking proposals to satisfy a need for RETIREE VISION BENEFITS FOR THE HEALTH OPTIONS PROGRAM ("HOP"). The Board is looking for a qualified insurance carrier to provide a fully insured, voluntary retiree vision program ( Vision Plan ) for Medicare-eligible participants1 who are in the HOP Medical Plan or the Value Medical Plan (and other similar plans as they may be implemented).
Background
The Public School Employees’ Retirement Board (the 'Board') of the Commonwealth of Pennsylvania is seeking proposals for a voluntary retiree vision benefits program as part of the Health Options Program (HOP).
The goal is to provide a fully insured vision plan for Medicare-eligible participants enrolled in HOP Medical Plan or similar plans. This initiative aims to enhance the health benefits available to retirees, ensuring they have access to necessary vision care.

Work Details
The contractor will provide a fully insured vision plan that includes:
1. Covered Services:
- Routine Comprehensive Eye Exam: 100% coverage in-network; $30 allowance out-of-network.
- Frames: $100 allowance in-network; $45 allowance out-of-network; PLUS Provider: $150 allowance.
- Lenses: 100% coverage for single vision, lined bifocal, lined trifocal, and lenticular lenses; progressive lenses have allowances based on tier.
- Contacts: $210 allowance if medically necessary; 100% coverage if medically necessary; $0 allowance for elective contacts.
2. Additional Services: Discounts on additional pairs of prescription eyeglasses, non-prescription sunglasses, and laser vision correction.
3. Customer Service Requirements: A toll-free customer service line available from 8 AM to 7 PM EST for inquiries regarding eligibility, billing, and claims.

Period of Performance
The contract will commence on January 1, 2026, and will remain effective through December 31, 2027. The Board has the option to renew the agreement on a year-to-year basis or for multiple years up to an additional three years until December 31, 2030.

Place of Performance
The services will be performed primarily in Pennsylvania as the program is designed for residents enrolled in the Health Options Program.

Bidder Requirements
The contractor must not be under suspension or debarment by any governmental entity and must maintain various insurance coverages including workers' compensation, general liability, professional liability, and fidelity coverage with specified minimum limits.
