Description

The State of Oregon, acting by and through the Oregon Department of Human Services, Office of Self-Sufficiency Programs, is issuing this Request for Proposal for Proposals. The purpose of these Services is to promote the health and well-being of refugees and other Federal Office of Refugee Resettlement eligible populations by providing opportunities to increase health literacy, coordinating health care, and organizing wellness groups. Background
The State of Oregon, through the Oregon Department of Human Services (ODHS), is issuing this Request for Proposal (RFP) to enhance the Refugee Health Promotion Services (RHP) program.

The RHP program aims to promote the health and well-being of refugees and other populations eligible under the Federal Office of Refugee Resettlement (ORR). The services are designed to increase health literacy, coordinate healthcare, and organize wellness groups for these communities. This initiative responds to the growing number of refugee arrivals in Oregon and the increasing health needs within these populations.

Work Details
The contractor will provide comprehensive services including:
- Health education classes aimed at increasing health literacy among ORR-Eligible Individuals.
- Coordination of medical and mental health services to assist individuals in navigating complex healthcare systems.
- Organization of wellness groups and peer support meetings to foster community connections.
- Development of individualized Plans of Care (POC) for each participant, ensuring that their specific health needs are addressed.
- Provision of interpreting services to facilitate communication with non-English speaking clients.
- Implementation of culturally responsive practices throughout all service delivery.
- Regular assessments to evaluate the effectiveness of educational programs and individual progress.

Period of Performance
The contract is expected to commence on April 1, 2025, and will continue until September 30, 2026, with potential extensions based on performance and funding availability.

Place of Performance
Services will be provided statewide in Oregon, focusing on counties experiencing the highest number of refugee arrivals, including Multnomah, Clackamas, Washington, Marion, Lane, Polk, Benton, and Linn.
