Installation of Carpet tile in office space

Background
NorthWest Senior & Disability Services (NWSDS) is issuing this Request for Proposals (RFP) to solicit qualified flooring contractors for the installation of carpet tile and repair work in their office space.
The goal of this contract is to enhance the flooring in Suite 190 of the Salem Annex Building, ensuring a professional and code-compliant installation that meets commercial standards.

Work Details
The contractor will provide all labor, supervision, tools, and related services necessary to complete the following tasks:
1. **Carpet Tile Installation**: Install approximately 7,100 square feet of Patcraft carpet tile from the Mid Century Mad Collection, specifically the Orbital style (Style Number: 10379, Color: Doo Wop #00770), with dimensions of 24” x 24”. Installation must adhere to Patcraft guidelines to maintain warranty compliance.
2. **Hallway Carpet Repair**: Perform localized repairs on approximately 120 damaged carpet tiles in the adjacent hallway, ensuring replacements match existing tiles in size, pattern orientation, and color.
3. **Cove Base Installation**: Install new 4-inch cove base made from rubber or vinyl commercial-grade material in areas affected by carpet installation. The color will be selected by the Owner prior to installation.

Period of Performance
The estimated project start date and duration of work are to be included in proposals; daily work hours are specified as 8:00 am to 4:30 pm.

Place of Performance
The work will be performed at the Salem Annex Building, Suite 190, located at 3414 Cherry Ave NE, Salem, Oregon.

Bidder Requirements
Bidders must provide proof of current insurance (general liability and workers’ compensation), an Oregon CCB license number, and identification of any subcontractors who will be utilized on the project.
All subcontractors must also show proof of current insurance and provide their Oregon CCB license number.
