Forklift Maintenance and Repair

Background
The Oregon Department of Administrative Services, Procurement Services (DAS PS) on behalf of the Oregon Department of Administrative Services, Enterprise Asset Management (DAS EAM) is issuing this Request for Proposal (RFP) for Forklift Maintenance Services. The goal of the contract is to obtain forklift maintenance and repair services for the state.

Work Details
The Proposer is required to submit a Technical Proposal including a current forklift operator’s license for maintenance and repair personnel, a detailed list of services intended to be provided to the State, a written plan for maintaining forklift maintenance records for each individual forklift, and a preventative maintenance and repair plan.
The plan should outline how Proposer receives preventative maintenance and repair requests, response time to confirm receipt of request and scheduling service, on-site response to urgent repair requests within 48 hours, detail the services provided as part of the preventative maintenance service, identification of personnel performing services with their certifications and licenses, outline the process of maintaining forklift records and how the Agency can access those records.
Additionally, documentation showing the years of experience in providing Forklift Maintenance Services is required.
