SODC Concrete Project

Background
The Southwest Ohio Developmental Center (SODC) is a Medicaid-certified Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), operated by the Ohio Department of Developmental Disabilities (DODD).
The goal of this contract is to install concrete in three specific areas at the SODC to enhance facility operations and safety.

Work Details
The contractor will be responsible for the installation of concrete in three areas:
1. **Shelter House**: A new concrete pad measuring 30’ x 12’ x 4” with a 4-inch gravel base, pinned to existing concrete, control joints placed as needed, and a broom finish.
2. **Administration**: Replacement of an existing concrete pad approximately 210 square feet, including removal and disposal of the old concrete. The new pad must have a 4-inch gravel base, be leveled with existing concrete, and finished with a broom finish.
3. **Administration Curb**: Replacement of a curb measuring 14’ x 12” deep, including removal of the existing curb and saw cutting to install the new curb.

Period of Performance
The project will begin within 30 days of issuance of the purchase order and must be completed during the hours of 7:00 A.M. and 4:00 P.M., Monday through Friday.

Place of Performance
All work will be performed at the Southwest Ohio Developmental Center located at 4399 E. Bauman Ln, Batavia, Ohio 45103.

Bidder Requirements
Contractors must maintain all necessary licenses, certifications, and registrations throughout the project duration.
All individuals providing services must pass a criminal background check and not have any offenses as outlined in Section 5123.08(1), (2), or (3) of the Ohio Revised Code.
Contractors must comply with prevailing wage laws and ensure that their employees do not possess illegal drugs or alcohol while working.
