Background
The contract is issued by the Ohio Department of Natural Resources, specifically the Division of Wildlife. The mission of this agency is to manage and protect Ohio's wildlife resources and habitats. The goal of this contract is to repair and replace the concrete floor at the Deer Creek Wildlife Area Headquarters Shop, ensuring a safe and functional workspace for wildlife management activities.

Work Details
The contractor will be responsible for the following tasks:

1. Remove and replace the existing concrete shop floor and apron.
2. Repair, level, and stabilize multiple cracks in the shop floor using polyurethane foam injection to fill voids and lift the concrete.
3. Drill 3/8 inch holes at strategic locations in the slabs for foam injection, followed by grouting and sealing these holes.
4. Repair any new cracks that may occur during the lifting process.
5. Seal all joints and preexisting cracks, grind down any unlevel areas that may pose tripping hazards.
6. Prepare subgrade by leveling, compacting, and installing a stable base for new concrete slab with rebar support.
7. Use fiber-reinforced concrete that is 6 inches thick with a smooth finish, which will also be sealed.

Period of Performance
The work shall be completed by June 30, 2025, for the crack repair phase and by October 30, 2025, for the full removal and replacement of the concrete floor.

Place of Performance
The work will be performed at the Deer Creek Wildlife Area Headquarters located at 12552 Post Road, Mt. Sterling, Ohio 43143.
