Background
The State of Ohio, through the Department of Administrative Services, General Services Division, is issuing this Invitation to Bid (ITB) for a Printing Term Contract under Category 47. The goal is to secure a contractor to provide Ohio Vital Records security paper forms for use by the Ohio Department of Health.

These forms are essential for health departments across counties and cities in Ohio to document proof of birth or death.

Work Details
The contractor will be responsible for printing several types of Ohio Vital Records security paper forms, specifically:
1. Death security paper (8.5” x 14” flat sheet) - approximately 1,000,000 sheets with an Intaglio border and specific alpha numeric numbering.
2. Birth security paper (8.5” x 11” flat sheet) - approximately 100,000 sheets with similar specifications.
3. Abstract security paper (8.5” x 11” flat sheet with full width micro perforation) - approximately 1,200,000 sheets with distinct features.

All forms must be printed on custom 28# white Fourdrinier watermarked security bond sheet with specific ink requirements. They must incorporate various security features including watermarks, fluorescent fibers, and chemical sensitivity. The contractor must also ensure that all forms are packaged securely and delivered to designated health departments across Ohio.

Period of Performance
The contract will commence on March 1, 2025, and will run until February 28, 2027. There is an option for renewal in one-year increments for a total contract life not exceeding five years.

Place of Performance
The products will be delivered to various health department locations throughout the State of Ohio.
