Prescription Labels

Background
The City of Cincinnati, through its Office of Procurement, is seeking bids for the supply of prescription labels, prescription vials, amber oval bottles, and custom printed paper bags. This procurement supports the Cincinnati Health Department's operational needs in providing essential pharmaceutical services.

Work Details
The contract encompasses the following items:

1. Prescription Labels:
- Thermal labels, 4" x 8", continuous roll.
- Approximately 320 labels per roll, 1" core, 2-color print on white background.
- Estimated annual usage: 670 rolls (approx. 214,400 labels).

2. Prescription Vials (Amber, Reversible Cap):
- Screw Top Designs in sizes: 11-Dram, 13-Dram, 16-Dram, 20-Dram, 30-Dram, 40-Dram, and 50-Dram.
- Thumb Tab Designs in sizes: 10-Dram, 13-Dram, 16-Dram, 20-Dram, 30-Dram, 40-Dram, and 60-Dram.

3. Amber Oval Bottles with Safety Cap:
- Sizes: 1 oz., 2 oz., 3 oz., 4 oz., 6 oz., 8 oz., 12 oz., and 16 oz.

4. Custom Printed Paper Bags:
- #8 size bags (3.5" x 1.5" x 10") – 45# paper weight;
- #16 size bags (5" x 2" x 10") – 45# paper weight;
- Various other sizes including bags weighing from 6 lb to 20 lb with specified dimensions.

Period of Performance
The initial contract period will last for twelve (12) months from the effective start date of the contract with options for two additional twelve-month renewals at the City's discretion.

Place of Performance
Delivery of materials will be made to the Cincinnati Health Department's Central Supply located at 1525 Elm Street, Basement, Cincinnati, Ohio.

Bidder Requirements
Bidders must be registered with the City of Cincinnati to submit a bid electronically through the Vendor Self Service website. The contract is subject to Living Wage provisions requiring minimum wage payments based on hours worked annually. Additionally, bidders must provide proof of insurance coverage as specified and comply with non-discrimination policies.
