Post Discharge Software RFP

Background
The Ohio State University Wexner Medical Center (OSUWMC) is seeking proposals for a post-acute care referral software solution aimed at expediting the referrals process and providing information to patients and families for better-informed healthcare decisions.

The goal is to enhance transitions of care services across various patient settings including inpatient, emergency department, and ambulatory care.

Work Details
The proposed software solution must:
1. Be adaptable to meet ongoing needs for improving transitions of care services for inpatient, emergency department, and ambulatory patients.
2. Provide up-to-date useful information to guide patients and families in decision-making.
3. Offer an integrated solution that sends and receives data from sources such as Epic EMR.
4. Comply with all State and Federal regulations including CMS and Joint Commission standards.
5. Meet standard healthcare data protocols.
6. Allow data reporting for post-acute referrals.
7. Provide tracking for at-risk patient populations (ACO population) in the post-acute space.

Period of Performance
The solicitation was released on October 1, 2024, with proposals due by October 22, 2024.

Place of Performance
The contract will be performed at the Ohio State University Wexner Medical Center.
