OHP 0060 HP7 Arrest Report 5 Part Carbonless Form 27602

Background
The State of Ohio, through the Department of Administrative Services, General Services Division, is issuing this Invitation to Bid (ITB) for the procurement of the OHP 0060 9/23 HP-7 Arrest Report (Uniform Traffic Ticket) Five Part Carbonless Form.
This contract aims to provide necessary documentation for use by the Ohio Department of Public Safety.

Work Details
The contractor is required to provide a total of 6,400 sets of the OHP 0060 9/23 HP-7 Arrest Report, packaged in four boxes containing 1,600 sets each.

The specifications for the forms include:
- Size: 4.25" x 10.25"
- Stock: Five-part carbonless; Part 1 (White CB, 15# bond), Part 2 (Canary CFB, 14-17# bond with carbonizing tissue), Part 3 (Blue CFB, 14-17# bond), Part 4 (Pink CFB, 14-17# bond), Part 5 (Buff CF, 33# ledger).
- All parts must print two-sided except for Part 2 which prints front only.
- Red crash consecutive numbering from C541601 to C548000 with no missing or duplicate numbers.
- Ink colors: Black and Red on all parts with close registration.
- Collation and perforation details are specified including a perforated stub at the top and tissue on Part 2.
- Finished trim size must be maintained at 4.25" x 10.25".
- Packaging requirements include shrink-wrapping sets in packs of 25 and labeling cartons with serialized ranges.

Period of Performance
The requested delivery date for the completed forms is by February 28, 2025.

Place of Performance
Delivery is to be made to the ODPS Alum Creek Facility located at 1583 Alum Creek Dr., Columbus OH 43209.
