Description

This RFP is being released for the purpose of contracting with up to two (2) Selected Offerors per region who will provide case management for the OHCW and SRS programs, which are administered and operated by ODM. All Selected Offeror(s) must be experienced in providing community long-term care case management services to children, adults, and seniors who have disabilities, are chronically ill, have medically complex conditions, and/or have behavioral health diagnoses. The Selected Offeror(s) shall support ODM's efforts by assisting in the implementation and management of these home and community-based programs and interfacing with individuals and providers at the local level to ensure individuals' health and welfare. The Selected Offeror(s)' responsibilities will include but not be limited to clinical and program management functions. MULTIPLE AWARD CONTRACT: This solicitation is issued to establish a Multiple Award Contract (MAC). A MAC is a Contract made with more than one Selected Offeror to provide the same or similar types of supplies or services for delivery within the same geographic area. This RFP seeks to establish a MAC with at least two Selected Offerors per region, and the Selected Offerors may be awarded a Contract that covers more than one region. The regions are defined in Supplement One: Region Map.
Background
The Ohio Department of Medicaid (ODM) is issuing this Request for Proposals (RFP) ODMR 2425-0010 for the Ohio Home Care Waiver (OHCW) and Specialized Recovery Services (SRS) programs. The purpose of this RFP is to establish a Multiple Award Contract (MAC) with up to two Selected Offerors per region to provide case management services for individuals with disabilities, chronic illnesses, and behavioral health diagnoses. ODM oversees Medicaid funding and aims to implement high-quality, cost-effective home and community-based programs.

Work Details
The Selected Offeror(s) will provide case management services for the OHCW and SRS programs, which include:

1. Clinical and program management functions.
2. Coordination with local providers and individuals to ensure health and welfare.
3. Support for the implementation of home and community-based programs.
4. Specific responsibilities under the OHCW include managing services such as adult day health centers, personal care aides, home modifications, and waiver nursing services.
5. Under the SRS program, responsibilities include Recovery Management and Individualized Placement and Support-Supported Employment (IPS-SE) services aimed at assisting individuals with Severe and Persistent Mental Illness (SPMI).
6. Collaboration with other ODM programs like HOME Choice, Ohio Benefits Long-Term Services and Supports, MyCare Ohio, etc.

Period of Performance
The contract period is expected to run from contract award through June 30, 2027, with the possibility of two renewal contracts extending through June 2031.

Place of Performance
Services will be provided across all 88 counties in Ohio.

Bidder Requirements
Offerors must meet several qualifications including:

1. Submission of a Technical Proposal and a separate Cost Proposal by the deadline.
2. Attestation of understanding all glossary terms in Section 1.4.
3. Inclusion of an Ohio certified Minority Business Enterprise (MBE) subcontractor for at least 15% of the total contract amount per state fiscal year.
4. Proof of experience in Medicaid operations or similar federally/state-funded programs within the last four years.
5. Key personnel must have specific qualifications including a Clinical Director who is a licensed RN or social worker with at least five years of management experience in community-based services.
