ODMR 2425-0010 OHCW and SRS RFP

Background
The Ohio Department of Medicaid (ODM) is issuing this Request for Proposals (RFP) to establish a Multiple Award Contract (MAC) for the Ohio Home Care Waiver (OHCW) and Specialized Recovery Services (SRS) programs. The goal is to contract with up to two Selected Offerors per region to provide case management services for individuals with disabilities, chronic illnesses, or behavioral health diagnoses.

These programs aim to support individuals in home and community-based settings, ensuring their health and welfare through effective management and implementation of services.

Work Details
Selected Offerors will be responsible for providing community long-term care case management services that include clinical and program management functions. Responsibilities encompass:
- Assisting in the implementation and management of OHCW and SRS programs.
- Interfacing with individuals and providers at the local level.
- Supporting ODM’s efforts in ensuring high-quality, cost-effective services.
- Collaborating with various ODM-operated programs such as HOME Choice, Ohio Benefits Long-Term Services and Supports, and MyCare Ohio.

Specific services under OHCW include: adult day health center, community integration, home care attendant services, personal care aide services, home modifications, waiver nursing services, among others. For SRS, services include Recovery Management and Individualized Placement and Support-Supported Employment (IPS-SE) for individuals with Severe and Persistent Mental Illness (SPMI).

Period of Performance
The contract period is expected to run from the award date through June 30, 2027, with options for two renewal periods extending potentially until June 30, 2031.

Place of Performance
The contract will be performed across all 88 counties in Ohio as defined in Supplement One: 'Region Map'.
