Background
The State of Ohio, through the Department of Administrative Services, is issuing this Invitation to Bid (ITB) for the purpose of obtaining a contractor to provide communicable disease charts for use by the Ohio Department of Health.
This initiative aims to enhance public health awareness and education regarding communicable diseases.
Work Details
The contractor is required to provide 6,500 units of the ODH-DCY Communicable Disease Chart. Each chart will be a 28” x 40” flat sheet printed on one side using a four color process. The charts must be scored at folds and folded down to a finished size of 14” x 10”.
Specific requirements include:
- Stock: 80# white gloss coated text
- Presswork: Print one side with no bleeds
- Ink: Four color process or full color copy (minimum 600 DPI)
- Bindery: Score at folds and fold to final size
- Proof: An exact color match PDF proof must be provided to designated contacts.
- Packaging: Charts must be carton packed and labeled with the title 'DCY08087 Communicable Disease Chart' and quantity.
Period of Performance
The delivery of the charts is requested by November 17, 2025, or sooner.
Place of Performance
The charts are to be delivered to the Ohio Department of Health, Bureau of Infectious Diseases, Attn: Sarah Mitchell, located at 246 N. High Street, 2nd floor, Columbus, OH 43215.
Note that receiving hours are from 8 AM to 3:30 PM EST.

This solicitation is a minority set-aside bid in accordance with ORC Chapter 125.08.1. Bidders must have an active MBE certification at the time the solicitation closes in order to submit a response.
