BMV 5745 Application Information for Ohio Driver License or ID Card. 32237

Background
The State of Ohio, through the Department of Administrative Services, General Services Division, is issuing this Invitation to Bid (ITB) for the purpose of obtaining a contractor to provide the BMV 5745 1/24 Application Information for Ohio Driver License or ID Card.
This contract is intended for use by the Ohio Department of Public Safety, specifically the Bureau of Motor Vehicles, which oversees the issuance of driver licenses and identification cards.

Work Details
The contractor will be responsible for printing 500,000 copies of the BMV 5745 1/24 Application Information for Ohio Driver License or ID Card.
The specifications include:
- Size: 8.5" x 11"
- Stock: 70# white uncoated offset (Laser Compatible)
- Presswork: Side 1 prints black ink only with screen; Side 2 prints black and process blue with screen, no bleeds.
- Packaging: Shrinkwrap at 100/package with chipboard backing.
- Delivery must include a packing slip detailing the name of the ordering agency, DAS job number, ODPS order number, quantity ordered, quantity shipped, requested delivery date, actual date of delivery, title of material, and any change orders.
- All cartons must be securely fastened to four-way pallets and meet specified pallet height and weight requirements.

Period of Performance
The requested delivery date for all printed materials is by August 29, 2025.

Place of Performance
The products will be delivered to the ODPS Alum Creek Facility located at 1583 Alum Creek Dr., Columbus OH 43209.

Bidder Requirements
Bidders must ensure that they do not insert a unit cost more than three digits after the decimal point in their bids.
Additionally, bidders are required to complete an Affirmation and Disclosure Form prior to contract award.
