BMV 5736 Power of Attorney for Ohio Vehicle Registration Form 34035

Background
The State of Ohio, through the Department of Administrative Services, General Services Division, is issuing this Invitation to Bid (ITB) for the BMV 5736 Power of Attorney for Ohio Vehicle Registration Form.
This contract aims to provide necessary documentation for vehicle registration processes managed by the Ohio Department of Public Safety.

Work Details
The contractor will be required to print 360,000 copies of the BMV 5736 Power of Attorney for Ohio Vehicle Registration Form.
The specifications for the form include:
- Size: 8.5" x 11"
- Stock: 50# Pink uncoated offset text
- Presswork: One-sided printing with horizontal micro perforation approximately 8-3/4" from the top of the page, no bleeds.
- Ink: Black
- Packaging: Shrink wrap in packages of 100.
- Delivery must include a packing slip with specific information including agency name, order number, quantities ordered and shipped, requested delivery date, actual delivery date, and title of material.
Cartons must be labeled appropriately and securely fastened to pallets for transport.

Period of Performance
The requested delivery date for all printed forms is by October 31, 2025.

Place of Performance
The printed forms are to be delivered to the ODPS Alum Creek Facility located at 1583 Alum Creek Dr., Columbus, OH 43209.

Bidder Requirements
This bid is designated as a minority set-aside in accordance with ORC Chapter 125.08.1.
Bidders must provide proof of being an Ohio bidder or having significant economic presence in Ohio or border states to qualify for a preference during evaluation.
