BMV 5736 Power of Attorney for Ohio Vehicle Registration Form 18665

Background
The State of Ohio, Department of Administrative Services, General Services Division, State Printing & Mail Services is seeking bids for the production of BMV 5736 1/24 Power of Attorney for Ohio Vehicle Registration Form. This form is intended for use by the Ohio Department of Public Safety.

Work Details
The contractor is required to produce 215,000 units of the BMV 5736 1/24 Power of Attorney for Ohio Vehicle Registration Form. The form should be printed one-sided on 8.5" x 11" 50# Pink uncoated offset text with black ink and no bleeds. It should include a horizontal micro perforation approximately 8-3/4" from the top of the page and be shrink-wrapped in packages of 100.

The specifications also include details on presswork, ink color, bindery, finished trim size, packaging, and required package labels. The contractor must provide a PDF proof to the Ohio Department of Public Safety for review.

Period of Performance
The requested delivery date for this contract is by June 6, 2024.

Place of Performance
The products are to be shipped to ODPS Alum Creek Facility, located at 1583 Alum Creek Dr., Columbus OH 43209.
