Work Requested:
Stony Brook University Hospital is seeking to purchase the Evoke spinal cord stimulation system and related items, which are manufactured by Saluda Medical Americas Inc. This procurement is for the supply of the specified commodity/product or service.

Requesting Entity:
The request is being made by Stony Brook University Hospital, part of the State University of New York (SUNY).

Background:
This purchase will be made under the hospital's discretionary authority, as permitted by NYS Education Law 355(5)(A), for procurements up to $125,000. No sealed bid process will be conducted for this procurement.

Contract Requirements:
The procurement is subject to NYS Finance Law 139 j&k, also known as the Lobbying Law. Vendors are reminded that all communications regarding this procurement must be directed exclusively to the designated Campus contact listed below.
Failure to comply with the contact requirements may result in consequences ranging from no contract award to debarment from sales to any New York State Agency.
The successful contractor will be required to complete and submit documentation related to:
   - Vendor Responsibility
   - NYS Finance Law 139
   - NYS Tax Law Section 5-a
   - Valid/current NYS Workers' Compensation and Disability Forms
