Background
SUNY Upstate Medical University is seeking to procure neurostimulators and related products manufactured by Nalu Medical. The goal of this contract is to provide essential medical devices that support the university's healthcare mission. Work Details
The procurement includes a variety of neurostimulator kits and accessories as follows:

- Complete System Kits:
- IPG + Lead(s)
- SCS Only - Dual 8 IPG + Dual 40cm leads Perm Kit (71019)
- PNS Only - Dual 8 IPG + Dual 40cm leads Perm Kit (71005)

- IPG and Lead Only Kits:
- SCS/PNS - IPG only Kit (dual 8) (71004)
- PNS - IPG only Kit (dual 4) (71020)
- SCS/PNS - IPG only Kit (single 8) (71003)
- PNS Only - IPG Only Kit (single 4) (71007)

- Lead Kits:
- SCS Only - 8 contact 40 cm lead Kit (72001)
- SCS Only - 8 contact 60 cm lead Kit (72002)

- Trial Lead Kits:
- 4 contact 40 cm trial lead Kit (72013)
- 8 contact 60 cm trial lead Kit (72003)

- Extension Kits:
- Extension 8 Kit (72004)
- Extension 4 Kit (72005)

- PNS Lead Kits:
- PNS Only - various configurations including tined leads of different lengths. Additionally, the contract includes user's kits, remote controls, chargers, adhesive clips, relief belts, limb cuffs, and various accessories such as needles, stylets, introducer assemblies, and tunneling tools.

Bidder Requirements
Vendors must comply with NYS Finance Law sections related to lobbying and vendor responsibility. The successful contractor will need to submit applicable documents related to Vendor Responsibility and New York State’s Tax Law Section.
