MVAP Medical - Respiratory Therapy, Sleep Diagnostics, Neurodiagnostic (EEG/PSG) Monitoring Supplies

Work Requested:
SUNY Upstate Medical University is seeking proposals for the supply of Respiratory Therapy, Sleep Diagnostics, and Neurodiagnostic (EEG/PSG) Monitoring Supplies. The scope of work includes, but is not limited to:

• Providing a range of supplies and products necessary for respiratory therapy
• Supplying equipment and consumables for sleep diagnostics
• Delivering neurodiagnostic monitoring supplies, including EEG and PSG items

Background:
SUNY Upstate intends to make an award under its discretionary authority as permitted by NYS Education Law 355(5)(a), for a value up to $125,000. This procurement is for the commodities and services described above. No sealed bid will be conducted for this opportunity.

Contract Requirements:
The successful vendor must:

• Comply with NYS Finance Law 139 j&k (Lobbying Law), meaning all communications regarding this procurement must be directed only to the designated contact by email
• Complete and submit all required documents related to Vendor Responsibility
• Submit documentation in accordance with NYS Finance Law 139 j&k and New York State's Tax Law Section 5-a
• Provide valid and current New York State Workman's Compensation and Disability Forms

Failure to adhere to these requirements may result in disqualification or debarment from future NYS Agency contracts.
