Description

Contractor shall provide patients with choice of shampoo, cut and style, trim, and perm/relaxer. The Facility will provide the shampoo and conditioner to be used. The Contractor will ask the patient what type of hairstyle he/she would like and shall provide that style. Patients may decline hair care services at any time. Services must be provided for a minimum of two (2) days per month.
