Bidders must submit a completed Vendor Information & Agreement form, Unpriced Proposal Forms, New Vendor Packet (including Vendor Registration Form and Conflict-of-Interest Disclosure Form), W-9 form, and proof of insurance coverage (Acord Form) with a minimum of $1 million in general liability insurance and $500,000 in workers' compensation insurance. Additionally, any vendor who will have contact with students must undergo a background check every two years.
