REQUEST FOR PROPOSAL (RFP) STATE PURCHASE-FY25-0183-SL, 1: Medical Telemonitoring Services for Fee-for-Service Participants

Background
The State of Missouri, through the Office of Administration, Division of Purchasing, is issuing this Request for Proposal (RFP) on behalf of the Department of Social Services. The mission of the Missouri Department of Social Services is to empower Missourians to live safe, healthy, and productive lives. The MO HealthNet Division administers the Missouri Medicaid Program and aims to build a sustainable Medicaid program that meets the needs of vulnerable populations. This RFP seeks competitive proposals for medical telemonitoring services for fee-for-service participants to ensure quality healthcare and minimize state expenditures through early intervention.

Work Details
The contractor shall provide medical telemonitoring services and equipment for eligible fee-for-service participants referred by the MO HealthNet Division. Key responsibilities include:

1. Providing in-home nursing visits as needed based on telemonitoring results;
2. Dispatching a nurse within 24 hours if potential health problems are identified;
3. Offering a toll-free support line for technical assistance;
4. Ensuring all personnel meet minimum experience qualifications;
5. Utilizing Class II FDA-approved medical telemonitoring devices, specifically the HomMed® Sentry Honeywell system or equivalent;
6. Collecting and transmitting vital signs data including blood pressure, heart rate, oxygen saturation, etc.;
7. Maintaining compliance with reporting requirements including monthly participation reports and quarterly evaluation reports;
8. Conducting background checks on personnel;
9. Ensuring all services are operational within 60 days after authorization.

Period of Performance
The contract period will be effective from the date of award through one year, with an option to renew for four additional one-year periods.

Place of Performance
Services will be provided statewide in Missouri, with specific participant interactions occurring at their homes.
