4. Compliance with minimum experience qualifications including five years of experience in community health partnerships and chronic disease projects.
5. Personnel qualifications for Direct Enrollment Specialists and HIV Case Managers must include relevant degrees or prior approval from the state agency.
6. Implementation of a health equity plan to ensure equitable access to care for all populations affected by HIV/AIDS.
7. Participation in monthly regional case management meetings and training programs as mandated by the state agency.
Period of Performance
The contract will be effective from the date of award through one year with options for renewal for two additional one-year periods.
