Background
The Minnesota Department of Human Services (DHS) is seeking to gather information regarding the definition of 'medically frail' for the implementation of new Medicaid requirements as mandated by Public Law 119-21, Section 71119. This initiative aims to ensure that individuals who qualify as medically frail can be exempt from work or community engagement requirements under the Medicaid program, effective January 1, 2027.

Work Details
The RFI focuses on developing a clinical definition of medical frailty and a claims analysis algorithm for identifying eligible individuals. The draft clinical definition includes two main categories: Specific Clinical High-Risk Medical Frailty Diagnosis Codes and Specific Social High-Risk Medical Frailty Codes.

Individuals will be considered medically frail if they meet certain criteria based on their medical history and diagnosis codes within specified time frames. The RFI requests feedback on the draft definition, clarity of criteria, evidence standards for verifying medically frail status, and considerations for access and equity in the application process.

Period of Performance
The RFI was posted on May 21, 2026, with responses due by June 1, 2026. The implementation of the new Medicaid requirements is set to begin on January 1, 2027.
