4. Establishing partnerships with local health departments and community organizations to enhance outreach and support.
5. Collecting data on referrals, screenings, and treatment outcomes to measure program effectiveness.
Period of Performance
The anticipated period of performance is from July 1, 2026, to June 30, 2027, with options for two additional one-year extensions (July 1, 2027 - June 30, 2028; July 1, 2028 - June 30, 2029).
Place of Performance
The contract will be performed in various jurisdictions across Maryland as specified in the applicant's proposal.
