Background
The Promoting Appropriate Transitions to Home (PATHways) investment program is initiated by the Health Policy Commission (HPC) to enhance collaboration between Aging Services Access Points (ASAPs) and qualifying Acute Care Hospitals in Massachusetts.

Work Details
The contract involves the following key components:
1. **Proposed Program**: Applicants must describe their proposed program, including staffing models, planned trainings for hospital staff, and communication plans between hospitals, ASAPs, and HCBS providers.
2. **Patient Population**: Applicants should detail the specific patient populations they intend to serve and how the program will enhance hospital services.
3. **Measurement and Data Reporting**: A workflow for tracking eligible individuals, offered services, and enrollment data must be outlined.
4. **Budget**: A detailed budget must be submitted that reflects all proposed expenditures for planning and implementation, with a maximum award cap of $210,000. At least 30% of total costs must be in-kind contributions from the awardee.
5. **Sustainability**: Applicants should describe how they will ensure the program's sustainability beyond the contract period.
