Concrete ramp resurfacing repair

Background
The City of Amesbury Department of Public Works (DPW) is issuing a contract for the resurfacing repair of a concrete wheelchair ramp at the City football stadium. The project aims to address safety concerns related to the ramp's current condition, which has deteriorated due to water infiltration and freezing, leading to structural issues.

The work must be completed before the High School Graduation on May 30, 2025, weather permitting.

Work Details
The contractor will be responsible for the following tasks:
- Remove and dispose of the existing membrane seal on the outdoor wheelchair ramp.
- Remove loose concrete that has separated from the substrate, approximately one inch in places.
- Apply self-leveling cementitious material to restore the surface.
- Reseal the ramp.

The total area affected is approximately 1,500 square feet. Additional drawings, sketches, and specifications may be provided as needed for bidding purposes.

Period of Performance
The planned start date for the project is May 12, 2025, with a contract duration until substantial completion of one week, contingent upon weather conditions.

Place of Performance
The work will be performed at Landry Stadium, located at 222 Main Street, Amesbury, MA 01913.

Bidder Requirements
Bidders must ensure that all workers have the required licenses and certifications. The following forms are required to be submitted with the bid:

- Certificate of Insurance
- Certificate of Non-Collusion
- Contractor Authorized Signatory Form
- Payment Bond and Performance Bond (50% payment bond if total contract exceeds $25,000)
- OSHA Certificates for all employees.
