Reagents and Collection Kits - LDH

Background
The Louisiana Department of Health - Office of Public Health is seeking to establish a Blanket Order Contract for the provision of Reagents and Collection Kits.
This contract aims to support the agency's mission in public health by ensuring the availability of necessary laboratory supplies.
The initial contract period will begin on July 1, 2025, or upon the date of award, and will last for one year, with the option to renew for two additional 12-month periods.

Work Details
The contractor will provide the following items under this contract:
1. Panther System (Brand: Hologic, Model #: 303095) - Quantity: 2 EA
2. Kit, Assay, Aptima Comb 2 (Brand: Hologic, Model #: PRD-05571) - Quantity: 240 KIT (250 Tests per Kit)
3. APTIMA SPECIMEN DILUENT KIT US (Brand: Hologic, Model #: PRD-03654) - Quantity: 1 KIT
4. Collection Kit, MultiSwab, Orange, Apt (Brand: Hologic, Model #: PRD-03546) - Quantity: 820 BOX (50 per Box)
5. Collection Kit, Urine, Yellow, Aptima (Brand: Hologic, Model #: 301040) - Quantity: 500 BOX (50 per Box)

All items are to be called for by the using agency as needed; no shipments are to be made until an order is submitted specifying the quantity required.
The unit prices should include any freight charges.

Period of Performance
The contract will be performed over an initial period from July 1, 2025 to June 30, 2026.
It may be renewed for two additional periods of twelve months each at the same prices if agreed upon by both parties.

Place of Performance
The products will be delivered to LDH Public Health Labs Virology located at 1209 Leesville Avenue, Baton Rouge, LA 70802.
