Background
This solicitation is to establish a Service Contract to provide Lead & Copper Sample Kit Delivery as specified for the Louisiana Department of Health - Office of Public Health for an initial period beginning date of award and ending June 30, 2024. At the option of the State of Louisiana and acceptance by the Contractor, this contract may be renewed for two (2) additional twelve (12) month periods at the same prices, terms and conditions. Total contract time not to exceed thirty-six (36) months.
Work Details
The contract requires the delivery of lead and copper sample kits, including coordination and delivery to prescribed sample locations (residents), as well as pickup of samples from each prescribed sample location and transport to LDH-OPH Laboratory.
