Thvc/Minimum Data Set Coordinator/Registered Nurse

Background
The Commonwealth of Kentucky is issuing this Request for Proposals (RFP) to establish a personal service contract for coordination, scheduling, assessment, care planning, completion, transmission, and validation of required resident assessments and minimum data sets at the Thomson-Hood Veterans Center (THVC).

The THVC is a 285-bed state veterans home located in Wilmore, Kentucky, providing skilled nursing services. The goal of this contract is to ensure compliance with state, federal, and VA requirements while utilizing PointClickCare software for electronic health records.

Work Details
The vendor will provide qualified licensed Registered Nurses (RNs) to complete the Minimum Data Set (MDS) and care plan processes for each resident at THVC. Responsibilities include: establishing assessment reference dates; creating an online calendar accessible to the interdisciplinary team (IDT); notifying IDT via email about assessment dates; managing calendars for OBRA Annuals and Quarterly MDSs; conducting initial and interim payment assessments; interpreting regulations related to interrupted stay situations; tracking Medicare beneficiary benefit periods; reviewing daily progress notes on Medicare residents; assisting in developing user-defined assessments within the Electronic Health Record; completing MDS nursing sections; transmitting completed assessments to CMS and VA; auditing care plans for accuracy; monitoring compliance with quality measures; providing monthly site visits; ensuring staff maintain current credentials and undergo background checks.

Period of Performance
The initial term of the contract is anticipated from April 1, 2025, through June 30, 2026. The Commonwealth reserves the right to renew this contract for up to two additional two-year periods.

Place of Performance
The services will be performed at the Thomson-Hood Veterans Center located at 100 Veterans Drive, Wilmore, KY 40390.
