Background
The Commonwealth of Kentucky is issuing this Request for Proposal (RFP) to establish a personal service contract for the Joseph “Eddie” Ballard Western Kentucky Veterans Center (WKVC).

The WKVC is a 156-bed state veteran’s home located in Hanson, Kentucky, providing skilled nursing services. The goal of this contract is to secure qualified coordination, scheduling, assessment, care planning, completion, transmission, and validation of required resident assessments and minimum data sets using PointClickCare software.

Work Details
The vendor will provide licensed Registered Nurse(s) to complete the Minimum Data Set (MDS) and care plan process for each resident at WKVC. Services include:

- Establishing assessment reference dates
- Creating an online calendar accessible to the interdisciplinary team (IDT)
- Managing admission tracking
- Overseeing OBRA Annuals and Quarterly MDSs
- Interpreting regulations
- Tracking Medicare eligibility
- Reviewing progress notes
- Assisting in developing user-defined assessments within the Electronic Health Record
- Completing MDS Nursing Sections
- Transmitting completed assessments to CMS and VA
- Auditing care plans for accuracy
- Providing monthly site visits
- Ensuring staff maintain current credentials
- Complying with confidentiality protocols under HIPAA.

Additionally, the vendor must provide two work references for candidates, copies of drug tests and background checks, tuberculosis test results, and ensure compliance with federal and state vaccination regulations.

Period of Performance
The initial term of the contract is anticipated from September 23, 2024, through June 30, 2026. There is an option to renew the contract for up to two additional two-year periods.

Place of Performance
The services will be performed at the Joseph “Eddie” Ballard Western Kentucky Veterans Center located at 926 Veterans Drive, Hanson, KY 42413.
