Dual Eligible Special Needs Plan (D-SNP)

Background
The Illinois Department of Healthcare and Family Services (HFS) is seeking to establish Medicaid Dual Eligible Special Needs Plan (D-SNP) contracts. The goal of this initiative is to provide Medicaid-covered services and Federal Medicaid Waiver services while minimizing cost increases to the state and ensuring a high level of care integration.

Work Details
The contract involves providing comprehensive healthcare services to individuals eligible for both Medicare and Medicaid. This includes care coordination, access to primary and specialty care, behavioral health services, and long-term services and supports.

The contractor will be responsible for ensuring that all services are integrated effectively to meet the needs of dual-eligible beneficiaries.
