Disposable Cutlery for CDC Warehouse

Background
The State of Iowa is seeking bids for Disposable Cutlery to be provided to the CDC Warehouse. The purpose of this Request for Bids (RFB) is to solicit bids from qualified providers to supply the goods and/or services described in this RFB to the Lead Agency and any Participating Agencies. The Lead Agency intends to award contracts beginning on the date listed in the solicitation, with options for annual extensions at their discretion.

Work Details
The contract includes the provision of various disposable cutlery items as follows:

1. DISPOSABLE, SERRATED DINNER KNIFE - Quantity: 1 Case (Last Year's Volume: 252 cases)
2. DISPOSABLE, DINNER FORK - Quantity: 1 Each (Last Year's Volume: 198)
3. DISPOSABLE, TEASPOON - Quantity: 1 Each (Last Year's Volume: 792)
4. DISPOSABLE, SPORK - Quantity: 1 Each (Last Year's Volume: 1,386 cases)
5. DISPOSABLE SHRINK WRAP - Quantity: 1 Each (Last Year's Volume: 0).

Period of Performance
The term of the contract will begin upon award and will be for one year, with an option for up to two annual extensions.

Place of Performance
The products will be delivered to the CDC Warehouse in Iowa.

Bidder Requirements
Bidders must comply with shipping terms of FOB Destination, Freight Prepaid. Additionally, bidders are required to submit a detailed quarterly sales report in Microsoft Excel on all sales made under the resulting contract and pay a 1.00% Administrative Fee on all sales made against the contract.
